When a Death Occurs

Our staff is available 24 hours a day, 7 days a week to respond to your immediate needs. When a death has occurred, call our office to speak to the director on call. He or She will answer any immediate questions and make arrangements to meet with you to discuss funeral preparations.

There are some important pieces of information you will need to provide upon meeting with your funeral director. Statistical information such as date of birth, social security number, occupation, etc. will be used to complete the certificate of death, which is a legal document that becomes a matter of public record. For your convenience, please print and fill out this Information Form and bring it with you when you meet with your funeral director.
